Healthy Communities
We are a committed organization that generates local, state, and national partnerships to address vital health needs in LaPorte County. We have turned a $25,000 designation into millions of dollars to provide health related services across LaPorte County for the last 16 years. We are unique in that we are the only Healthy Communities in Northwest Indiana and one of a few in the entire state. Beginning as an initiative in 1996 we have expanded to include tobacco cessation programs and have worked towards insuring kids and families. We have two coalitions, Health Coalition and Tobacco Prevention & Cessation Coalition. They are both composed of health care providers, social services, wellness organizations, and employers representing focused on making LaPorte County a healthier place.

Covering Kids & Families
Covering Kids & Families of Indiana, Inc. (CKF-IN) is a statewide organization committed to ensuring that all children and families eligible for Medicaid and the State’s Children’s Health Insurance Program (SCHIP) are enrolled.

Healthy Communities Coalitions
The Healthy Communities Coalitions are focused on improving the health of LaPorte County and surrounding areas. The coalitions bring together local community organizations to advocate for a healthier community. The coalitions work to address issues regarding health coverage, tobacco prevention and cessation, and more.
The coalitions offers:
- Networking opportunities for professionals
- Organizations may promote their programs and events
- Volunteer opportunities to improve the health and well-being of NWI residents

Tobacco Prevention and Cessation
The Tobacco Prevention and Cessation Commission from the Indiana Department of Health has afforded HCLC the opportunity to collaborate with many local healthcare providers, employers, and organizations on innovative prevention and cessation projects throughout LaPorte County.
